AHOW Baby Dedication Request Form

Please read the following instructions carefully:

The form must be completed at least 3 weeks before your preferred. When filling the form, please make sure you select your preferred date and an alternate date. If your preferred date is not available for any reason, you will be giving an alternate date you chose.
Note that families and friends will not be allowed to dance forward with the parent as it was previously done. Only the father and the mother will dance forward with their baby being dedicated.
Please be aware that because baby dedication is done once a month, it is most likely that multiple babies will be dedicated each time. However, each child will be prayed for and dedicated to the Lord separately.
Parents who want a private baby dedication should not fill this form but must contact the church office at info@agapehousenj.org. This can be done during the church office hours on a day other than Sunday.
All baby-dedicating parents must attend a baby dedication forum (via zoom) that will be offered before the Sunday of the dedication. The forum will include teaching instruction and prayer to ensure the parents are adequately prepared for this important process. You will be contacted after the signup.
Failure to attend the forum will lead to the postponement of you baby dedication.
